The Mansion Poker Dome Rules and Questionnaire

Since a few of the participants in the KAP online league asked, I thought I'd post the rules and questionnaire for the MansionPoker Poker Dome Challenge. First of all, here's the questionnaire, which the show's producers --- Hi, Adam! --- use to think up all sorts of space-filling questions for those "personality" filler shots you see on the program.

Okay, now here's the official rules thingy that you have to sign, and yes, 21 is the minimum age --- sorry to all you 19- and 20-year-olds out there!


1. Your date of birth. You must be 21 years old to participate.
2. Male or female?
3. Your name as it appears on your passport
4. Address including county or province, country
5. Phone numbers: day, home, cell
6. e-mail address (all communication and airline tickets will be by
e-mail)
7. Your complete name including given name and married name; your
mother's given name.
8. Where you were born: include city, county, state or province,
country
9. Driver's license number and state/country where issued
10. Passport number and country issuing
11. Social Security Number
12. Country you are a citizen of:
13. If you have lived at your current address for less than 7 years,
list your complete previous addresses and the dates you lived there.
14. Nearest major or international airport(s) to your home
15. Your jacket size (for your Mansion Pokerdome jacket): M/W,
Small, Medium, Large, Extra Large


And some additional info, from a separate mailing:

1. Telecast CLOTHING: men-collared shirts, jackets, ties
optional--business attire. women--dresses or suits--smart business
attire. No white clothing, shorts or logos. Please contact me regarding
any questions on clothing if you have not already done so.
2. Caesars Palace check-in: Please check in at the Emperor's Salon VIP
room--big double doors to the left of the main hotel registration area.
You'll be staying in one of Caesar's deluxe tower rooms. For security
purposes and for any incidentals you may wish to charge, Caesars Palace
requires all guests to post a credit card upon check-in. Again, if you
do not have a credit card, you will be required to give a $200 cash
security deposit (refundable upon checkout.)
3. Weekend itinerary (times subject to change):
Friday afternoon interviews—a Fox production staff member will contact
you, or contact information will be included in your welcome letter
upon hotel check in. Friday afternoon, leave Caesars Palace (Augustus
Tower entrance) 4:30 p.m., you will be taken to the Poker Dome. The
production dinner will follow immediately, at 8 p.m.
Saturday: 3:30 p.m. leave Caesars for Poker Dome—rehearsals, followed
by show taping at 8:30 p.m.
Sunday: hotel check out. VIP transportation to airport—be at Augustus
Tower entrance 2 hours before your flight departure time.
4. Friday Night Dinner: For players only please. We're happy to arrange
for your adult companions to view the telecast taping on Saturday.

This probably a standard schedule, more or less. The VIP transportation is one of those black charter vans as seen in the photo below. Damn things were hot in the desert sun!

Mansion has banned US players.

Not sure if its only the casino and sportsbook but you might want to check up on it.

Hiya, drizz!

Still open for poker is the latest word that I have. Closed to sportsbook and casino gaming (to U.S. players), so for those of you looking for a repeat of that $1,000 Steelers freeroll, it's not going to happen.
